Protecting your privacy

In accordance with the Foundation's values, we respect the individual worth and dignity of all people. The Foundation is committed to achieving best practice by adhering to the Privacy Act.

1988 (Cwth) (Privacy Act). The Privacy Act sets out the principles concerning the protection of your personal and health information, the National Privacy Principles (NPPs) and the National Health and Medical Research Council (NHMRC) guidelines approved under s95A of the Privacy Act (NHRMC Guidelines).

The Foundation is primarily a research based not for profit service, incorporated as an Association in accordance with the Associations Incorporation Act 1987 (WA).

The Objects of the Foundation are to establish a research fund, to promote and provide financial assistance for the facilities and trained personnel for research into the cause(s), diagnosis, prevention and treatment of Alzheimer's disease, including but not limited to:

  • advancing the knowledge of the cause(s) of Alzheimer's disease and related disorders both familial and sporadic;
  • research to identify therapeutic agents that prevent or retard the progression of Alzheimer's disease;
  • fostering research into the development of new techniques and procedures that would aid in the early diagnosis and treatment of Alzheimer's disease.

The Foundation provides a range of research, fundraising and other services to fulfill its Objects and Vision.

Your personal information

The Foundation's Privacy Policy applies to information about individual's personal, health and other relevant information. This can include your name, date of birth, address, telephone number, e-mail address, bank or credit card details, health and disability status, services provided and other personal or relevant information collected to undertake research or provide a health service.

The only time the Foundation collects, uses or discloses your personal, health and other relevant information is when it is necessary either to provide a health service to you, when it is allowed by law, or when you have given us your consent.

Why we collect this information

We collect this information to undertake research, provide health services and to meet the government reporting requirements.

How your information is collected

The Foundation collects information in a number of ways, including:

  • Directly from you when you provide information by phone, in person or in documents we ask you to complete;
  • From others who have information relevant to our ability to undertake research and provide a quality health service to you. This information would for example be sought from doctors, other health professionals and your family members.

How we use your information

Your personal and health information may be used in order to:

  • Undertake research into Alzheimer’s disease;
  • Develop the Foundation’s services;
  • Keep you informed of new developments within the Foundation;
  • Contact you about the Foundation’s fundraising activities; and
  • Disclose information to a third party.

When the Foundation does share your information with a third party it is for the purpose of assisting us to undertake research or to provide a health service that is relevant to your needs. Third parties to whom we would disclose your personal and health information include health professionals and organisations involved in your health care or associated with Alzheimer’s disease research.

Consent to collect and use your information

The Foundation will ask for your consent for the collection, use and disclosure of your information or for secondary use and disclosure of your information, where practicable, through the use of a consent form specific to the purpose intended.

Help us to ensure we hold accurate information

The Foundation is committed to taking all reasonable steps to ensure that the personal information we collect, use and disclose is accurate and complete. However, the accuracy of that information depends to a large extent on the information you provide.

We would appreciate if you could:

  • Let us know if there are any errors in your personal information; and
  • Keep us up to date with changes to personal information such as your name and address.

Using government identifiers

In certain circumstances we are required to collect government identifiers such as tax file numbers, Medicare numbers, pension numbers or Veteran's Affairs numbers. The Foundation does not use or disclose this information other than when required or authorised by law.

You can access your personal information

You have the right to access your personal information held by the Foundation however, your access may be subject to some exceptions imposed by law. If you would like to have access, please let us know. For security reasons, you may be asked to put your request in writing.

Changes to our privacy policy

The Foundation may make changes to this Privacy Policy from time to time for any reason. We will publish those changes on our website.

We recommend that you review our Privacy Policy whenever you visit our website in order to obtain the most current statement of our Privacy Policy.

Anonymity

When you contact the Foundation in relation to certain specific matters, such as your participation in a clinical research trial, it may be necessary to identify you. However, if you were making a general inquiry about our services for instance, we offer you the opportunity to do this anonymously.

If you are not happy

If you believe that the Foundation has not acted in accordance with the Privacy Act 2000, you may lodge a complaint with the Executive Officer in person, by phone, fax, e-mail or in writing. We will act quickly and courteously to resolve any complaints you may make.
